Cancellation/Late Policy
It is possible to cancel an appointment, via the confirmation email, up to 24hours in advance of the appointment time. Without 24 hour notice of cancellation, the client will be charged 50% of the full fee. If the client shows up late within the first 15 minutes of the session, a shortened session will be given; if the client shows up more than 15 minutes late, the session will be terminated and the client will be charged 50% of the fee for the session.
Payment
We accept cash, check, and credit card payments. Payment is expected at time of service. A $30 fee will be charged for returned checks. Cash or check is preferred. A rate increase of roughly 10% for base rate (one hour) pricing is to be expected annually. This is an estimate and this may change depending upon the course of therapy and the changing needs for your treatment.
Scope of Practice
In the event that the needs of the client should surpass my scope of practice, information or referrals will be provided to assist the client accordingly.
Client/Therapist Relationship
These sessions are therapeutic and non-sexual in nature. If the client violates this part of the professional relationship the session will be immediately terminated and the client will not be reimbursed.
Confidentiality/HIPAA
All health and personal information given during a session is confidential; if an email address is provided it will automatically be added to the email distribution list [less than 3 emails a month]. Client information will only be shared with a third party if:
-information regarding child abuse has been disclosed
-legally subpoenaed
-the client has given written consent
Being a healthcare provider, J9Massage & Beyond will not disclose any medical/health or personal information without written permission from the individual client, including disclosures between other healthcare practitioners. Additionally, the medical/health or personal information of healthcare providers at, or affiliated with, J9Massage & Beyond is equally protected and individuals may choose to disclose information at their own discretion.
In an effort to respect medical privacy, please do not ask our healthcare providers about their personal medical history, circumstances, or treatments, including vaccinations.
